We are excited to help you maintain your dental health and take pride in the relationship that is developed with each patient. Our goal is to offer quality and affordable dental care while maintaining a high level of administrative efficiency.
We recommend printing and completing our new patient forms prior to your visit as they do take a few minutes. If you have any problems accessing the forms, call our office and we will gladly email them to you.
These forms will cover your medical and dental history, dental insurance (if applicable), as well as our office HIPAA and financial policies. If there is pertinent information that we should know prior to your appointment (required premed, specific allergies or requests) please let our office know before you arrive. We reserve two hours for all new patient cleaning appointments to ensure a thorough visit; please allow for that time on your schedule.
